Insurance Information
We are contracted with most preferred provider organizations (PPO) and most health maintenance organizations (HMO). We also accept Medicare. Please call our office at 925.754.6767 to see if your plan is one we accept.
We bill your insurance for your visit, except for services considered cosmetic. We are required by your insurance company to collect the co-pay, co-insurance, and deductibles at the time of your visit. Visa and Mastercard are accepted for your convenience. All insurance contracts are between the patient and insurance company. As a result, the patient is responsible for his or her bill.
Informing us of any change in your insurance coverage is critical. Failure to do so may result in your being responsible for any charges.
All HMOs and some PPOs require patients to obtain referrals and/or authorizations from their primary care physician (PCP) prior to consultation and treatment. Our contractual obligations with these insurance plans prevent us from seeing any patient without such authorizations.
If you have a credit balance we shall be happy to refund that amount or apply it to your next visit. Just let us know what you prefer.
We understand that dealing with insurance providers can be most trying for all concerned. Patience, perseverance, and at times, a sense of humor are essential.
